Thursdays 1:45 PM - 4:00 PM
What you need to apply for a passport:
1. Passport Application (Form DS-11)
2. Proof of U.S. Citizenship (Certified Birth Certificate, U.S. Passport, Consular Report of Birth & Naturalization Certificate)
3. Copy of front and back of Driver's License
4. Two Identical Passport Photos
Correct Fees for Passport Book:
16 years old & up: $75.00
Under 16 years of age: $60.00
Expedite Service: Additional $60.00 per passport book/card
Correct Fees for Passport Card:
16 years old & up: $20.00
Under 16 years of age: $10.00
Check or Money Order only made payable to "U.S. Dept. of State"
Separate Agency Fee of $25.00 (cash, check, or credit card)...Check made payable to "Town of Dartmouth"
