Monday thru Friday at 9:30 am seniors who are on the reassurance list will be called. During the conversation if any concerns are brought to light the Outreach worker will be notiifed to take further action.
Information needed is seniors name address and telephone number. Emergency contact would be most helpful.
Family members may set this in place for their senior family.
