Our Mission
The Dartmouth Adult Social Day Program is committed to helping the elderly community who are homebound or in need of supervision, to continue living independently or with caregivers. Socialization and supervision is provided in a welcoming, structured, and supportive environment while providing respite to caregivers.
Program Goals
- To provide a safe, structured, caring and nurturing environment
- To provide a program of activities that create mental, social and physical stimulation
- To preserve emotional health and independence
- To overcome isolation
- To provide respite to caregivers
- To delay the need for long-term nursing home placement
Eligibility Criteria
- Elder must be able to administer medication independently throughout the day
- Elder must be able to eat independently
- Elder must be able to ambulate, use restroom ,and transfer without hands-on assistance
- Elder must be able to enter and exit the Social Day vehicle without any hands-on assistance
- Elder is no longer appropriate for a Social Day Program if he/she:
- displays verbal or physical behavior which endangers staff, other elders, or themselves
- Requires hands on assistance or a level of service beyond the scope of a supportive day program
