Getting a Library Card
How to Get a Library Card
Any Massachusetts resident may apply for a SAILS library card at the Dartmouth Public Libraries. The first card set is free, replacement sets are $3. Out-of-state residents may apply for a restricted-use, nonresident card, which allows access only to the collection of the library at which the card was issued, and not to the entire SAILS network.
Library cards are not transferable. The user is responsible for all materials checked out on his or her card. If a card is lost or stolen, it should be reported to the Library immediately.
Obtaining a Dartmouth Library Card
Adults: Ages 18 & Up
Patrons over the age of 18 may fill out an application in person at the library or print out, fill it in and bring it to the library. Download the Library Card Application (PDF) to print out and bring in.
All applicants are required to obtain their physical library card in person. A picture ID and proof of address is required. Accepted forms of picture ID are:
- Valid driver's license
- Passport
- Council on Aging ID
- State identification card
- School identification card
If one of these forms of ID cannot be presented, optional forms may be accepted by the Library Director, Reference Librarian or Head of Circulation.
All applicants need proof of address either from the picture ID or via another form. Accepted items for proof of address include:
- Utility bill
- Lease
- Official correspondence with name and address printed on it
If all identification is approved and signature obtained, the library card will immediately be issued and full borrowing privileges will be granted. In signing the application form, the patron agrees to "…be responsible for material borrowed with the card, for all the fines incurred, and for loss and damage of material charged upon the card. The patron releases the library from any liability for damages occurring from equipment or material borrowed"
Children's Cards: Through Age 12
The Dartmouth Public Libraries encourage any child who can write their name to apply for their own library card, regardless of age. Children age 5 and up are eligible for a library card whether or not they can sign. Children 12 and under need the signature of a parent or legal guardian in order to receive a library card.
In signing the application form, the parent or legal guardian agrees to "…be responsible for material borrowed with the card, for all the fines incurred, and for loss and damage of material charged upon the card. The patron releases the library from any liability for damages occurring from equipment or material borrowed. They accept responsibility for the selections of materials made by this person. This may include access to electronic resources such as the Internet."
Children's Cards: Ages 13 Through 1
Children, ages 13 through 17, who have picture identification may obtain a library card without the signature of a parent or guardian. Children who do not have identification may have a parent or guardian show acceptable ID and verify the child's ID and address.

The Dartmouth Library Staff will make accommodations for children and/or parents or guardians who are disabled. Please contact the head of Circulation or the Library Director if any accommodations are needed.
