Due 30 Days Prior to the Scheduled Event
This information is intended for any individual or organization that is planning on holding an "event" such as, but not limited to, potluck events, car shows, bizarre, and festivals.
Coordinators for events should first read through our Guidance for Events or Potluck Events (PDF) to determine what their next steps should be. If the event is not a charitable or religious event as explained, coordinators should use the below guide for complete information on the application and permitting process.

In accordance with the provisions of State Sanitary Code Chapter X, 105 CMR 590.010(A)(2) a and b, each caterer that plans to serve a meal in Dartmouth shall give written notice to the Dartmouth Board of Health prior to or within 72 hours after serving the meal. We ask for the following information:
- Name of Caterer, Address, Business Phone
- Date, Time, and Location of Catering Event
- Person in Charge
- Number of Meals to be Prepared in Commissary or Establishment
- Menu
- Copy of Food Service Establishment Inspection and Permit
- ServSafe Manager Certificate of the on-site manager
- Allergen Awareness of the on-site manager
